Wellness screening results are snapshots of recent symptoms, habits, or perceived load, not diagnoses. Record the timeframe used by the questions, whether answers describe a typical week or an unusually difficult day, and any major change in sleep, work, medication, pain, or support. A score becomes more useful when repeated under comparable conditions rather than treated as a permanent label.

Interpret the pattern alongside functioning and safety. A low-looking score does not rule out a serious concern, and a high score does not identify one cause. Wellness screening formulas

PSS-4 score = sum of four item ratings (0–4 each). Range 0–16. Norms: low ≤ 5, moderate 6–10, high ≥ 11. The scale includes two positively worded reverse-scored items.

Sleep debt = recommended sleep − actual sleep (hours per night). Cumulative debt = nightly debt × nights under-slept. Recovery nights ≈ cumulative debt / 1.5 h (estimated extra sleep per recovery night).

Pain intensity: NRS 0–3 = mild; 4–6 = moderate; 7–10 = severe. Duration: < 7 days = acute; 7–84 days = subacute; > 84 days (12 weeks) = chronic.

Interpreting wellness screening results

When to seek professional support

A PSS-4 score of ≥ 11 (high stress) over a sustained period is associated with increased risk of burnout, anxiety disorders, and cardiovascular events. If you consistently score in the high range, speaking to your GP or an occupational health professional is advisable. Chronic sleep debt (> 10 cumulative hours) is associated with impaired immune function, metabolic dysregulation, and significantly reduced cognitive performance. Chronic pain (> 12 weeks) requires professional assessment — the BPS biopsychosocial model recognises that chronic pain involves biological, psychological, and social factors beyond simple tissue damage.
